Gulani, MD, submitted a flurry of post-radial keratotomy (RK) videos, each with a unique twist. Using a Sealant to Bond RK and Corneal Incisions

Dr. Gulani highlights two scenarios in which he uses the ReSure Sealant (Ocular Therapeutix) to bond RK and corneal incisions. In the first video, he presents the case of a deep 12-incision RK with advanced Fuchs dystrophy and a grade 4 cataract with high hyperopia and astigmatism. Dr. Gulani uses the sealant to close the main incision and reinforce the proximate RK incision.

The second video features a dense cataract case with RK incisions. Dr. Gulani uses a secondary instrument to feed lens material to the phaco tip to avoid distorting the RK incisions and uses a toric lens manipulator to move the IOL into place; however, as the case is coming to a close, he notices the RK incision nearest to the main incisions is slightly splayed, so he applies the ReSure Sealant to the RK and main incisions.

Placing Iris Hooks in Post-RK Cataract Surgery

Here, Dr. Gulani describes a post-RK cataract surgery case in a patient with a poorly dilating pupil due to years of pilocarpine use. The iris hooks are placed strategically around the RK incisions.

RK in a Retina Case

This video presents an RK case with prior vitreoretinal surgery and macular degeneration. Dr. Gulani utilizes a metal blade to keep the RK incisions blunt and describes other methods he employs to limit stress on the capsule and the incisions.
